55 Man, BMI 47, needs a laparoscopic cholecystectomy.  PMx: IDDM, HTN, asymptomatic CAD on imaging, snoring, neck size 21.  Stable on meds, passable functional status, mows the lawn on weekends (riding!). Where do you do the surgery?
